Yearly Membership Fee
First Family Member – $325/Year
Add. Family Member – $275/Year
– 2 Routine Dental Cleanings
– 2 Routine Oral Exams
– 1 Emergency Examination
– 1 Set of Digital X-rays
– 1 Fluoride Application (children up to 18 yrs only)
– 15% Discount on all dental treatment
– No Yearly Maximum
– No Yearly Deductible
– No Annual Waiting Periods
– Membership begins the day of enrollment, and is effective for one year.
– Membership dues are payable in full upon enrollment, and are non-refundable.
– Cash, check, or credit cards accepted.
– Fees for memberships and dental services are subject to change annually.
– Membership is non-transferable.
– Membership cannot be used in conjunction with any other dental insurance or dental plan.
– Membership is NOT insurance, and can only be used at Country Club Dental Care.
– Payment is due when services are rendered.
– It is the responsibility of the patient to maximize their benefits by arranging appointments within the 1 year membership period. If benefits are not used within the membership year, refunds will not be issued.
